Include the action, approximate UTC time, visible error, and license or final identifier. Redact private media, email codes, credentials, and download links from screenshots.

If an action's outcome is uncertain, refresh its record before retrying. A missing response does not mean nothing happened.

For a download, check browser Downloads first. A browser handoff does not prove a saved file. Downloading an already released final does not start another generation or occupy another final slot.
